Upon booking, you must complete the consent, medical, and dental forms for you or the person(s) you are booking for. Please save a card on file to reserve your appointment(s). You will not be charged unless the appointment policy is violated.
I cannot accept your appointment request until the forms are completed.
There is a consent form outlining the expectations of reserving an appointment. By reserving your appointment, we are setting time aside specially for you and/or your loved ones. I kindly ask you remember there is often a waitlist of patients needing sooner availability. By not showing up to this reserved appointment, the opportunity is taken from someone in need. If you cancel or no show without 48 hour notice, you will not be able to rebook with Seaside Dental Hygiene Studio for a new patient visit until a non-refundable deposit is paid in full.
